SCAN Health Plan is a not-for-profit organization dedicated to providing comprehensive Medicare Advantage plans to seniors, aiming to keep them healthy and independent. Established in 1977, SCAN has grown to become one of the largest not-for-profit Medicare Advantage plans in the nation, serving over 200,000 members across California. The organization offers a range of health plans tailored to meet the diverse needs of older adults, emphasizing personalized care and community engagement.
What We Do
SCAN Health Plan offers a range of services to support seniors, including:
- Medicare Advantage Plans: Tailored plans, including a Fully Integrated Dual Eligible Special Needs Plan for individuals eligible for both Medicare and Medi-Cal.
- Care Coordination: Personalized support to help members manage their healthcare needs.
- Health and Wellness Programs: Includes fitness classes, nutrition counseling, and disease management.
