    In association with TLC Home Hospice of Moorpark, Loving Heart Hospice Foundation supports terminally ill patients in Ventura County and San Fernando Valley.

    Description:
    Loving Heart Hospice Foundation identifies and trains volunteers to visit TLC Hospice patients in their homes to provide social interaction and perform simple tasks that help the patient and/or their family.

    In addition, we hold fundraising events and programs that raise money in order to provide financial aid to needy patients and/or their families. Such help focuses on practical needs such as caregiver relief, cremation or funeral expenses, and days in a skilled nursing facility.

    We also provide three FREE Grief Counseling Sessions in Ventura County: one twice a month at the Moorpark Adult Center, one twice a month at the Los Robles Hospital East Campus, and one every week at the Simi Valley Senior Center.

    We often use volunteers in our office to help with clerical and general office duties. High school students seeking community service hours are a great help to us.

    History:
    TLC Home Hospice was established in 1994 as a Medicare/MediCal certified and JACHO-acredited medical hospice service provider.

    When TLC Hospice realized that many terminally ill patients in Ventura County and San Fernando Valley were financially needy and struggling to pay for hospice expenses, Loving Heart Hospice Foundation was established in 1996 and received 501(c)(3).
